For clinical hypnotherapy, fertility support & resolving natal trauma clients, following your free telephone consultation you will receive an initial consultation questionnaire.  Completing this provides you with the opportunity to elaborate further about the changes you wish to make & your specific set of circumstances and answer basic questions about your current health and situation.

Returning this prior to your first session with me means that I have a better idea of  how we can begin to work together to help you make the shifts you desire before we even meet for your first appointment - in short, it allows us to have a head start in our first session together.
